- GENERAL WAIVER OF LIABILITY

I understand that dance and exercise programs require physical exertion and that physical injury may result from participating in such activities. Despite this risk, I am voluntarily participating in such activities and I voluntarily elect to assume all risk involved.

In consideration of my participation, I release and hold harmless Katelyn Elison, doing business as Ascend Dance Studio, its owners, administrators and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Ascend Dance Studio, its owners, administrators, and instructors or in route to or from any of said premises. Further, I release and hold harmless Ascend Dance Studio, its owners, administrators and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by any non-student/non-participant(s) who may accompany me, while in or upon the premises or any premises under the control and supervision of Ascend Dance Studio, its owners, administrators, and instructors or in route to or from any of said premises.

- MEDICAL RELEASE

I hereby certify that the participant is in proper physical and mental condition to participate in the enrolled activities. I agree to notify Ascend Dance Studio immediately and in writing if there is any change in the participant's condition. I have disclosed any and all physical or mental issues, restrictions, or conditions regarding the participant to Ascend Dance Studio. I give Katelyn Elison, doing business as Ascend Dance Studio, its owners, administrators and teachers permission to seek emergency medical treatment for the participant and I request that the participant be transported to the nearest hospital if necessary. Ascend Dance Studio will contact me and/or an emergency contact I provide them at the earliest opportunity. I understand that Ascend Dance Studio cannot dispense any form of medication.

- PHOTO RELEASE

I understand that from time to time Katelyn Elison,, doing business as Ascend Dance Studio participants will be photographed and/or videotaped while participating in any activity. I further understand that these photos and videos may be used for appropriate promotional purposes for Ascend Dance Studio or may be provided to, or purchased by, participant's friends and family to be kept as treasured mementos. I hereby authorize Ascend Dance Studio and any Ascend Dance Studio hired photographers and videographers to take photographs of and videotape participant at recitals and other similar types of performances, including practices for such performances. I hereby waive any right to inspect or approve any image or use and grant Ascend Dance Studio permission to copyright such photographs and video in its name. Participants' last names will not be used on the Ascend Dance Studio website or in Ascend Dance Studio print advertising.
